甲方有意聘用乙方为外籍教师，乙方有意受聘并接受甲
方所提供的基本待遇。双方本着
平等自愿，协商一致的原则，达成以下本意向书：
一、工作内容：甲方拟聘用乙方从事
英语教师

四、保险：乙方持工作签证并与甲方签署正式聘用合同
后，甲方为乙方购买人身意外和
公共交通意外保险。乙方获得当地公安机关颁法的《居留
许可证》后的下一个月起，甲方为

住院医疗保险和工伤保险）

社会保险卡将由
政府职能部门于乙方取得
《居留许可证》

个工
作日左右发放给乙方
（节假日顺延）
并开
始持卡享受中国国民一样的上述社会保险待遇。乙方如需就医，须在社会保险定点医院就医
并按社会保险规定承担相应比例的费用，在非定点医院就医费用自理；受聘方的挂号、就医
< br>交通、镶牙、整容、保健按摩、配眼镜、住院就餐和服用非医疗性的滋补药品以及医生出诊
等费用自理。

glasses,hospitalized
dinning and
taking of
nutritious food as
well as appointing doctors.
五、其他条款以报到后签订的正式合同为准。

六、注意事项：

乙方在签署聘用意向书后应该立即准备以下文件
给甲方用于办理签证相关文件：

乙方需在签署意向书后

延误，造成无法如期办证，责任自负。

乙方来中国时，

带上最高学历证书原件、
教师资格证原件以及无犯罪记录原件

通过政府职能部门的审核。若无法提供原件或遭政府职能部门拒绝，甲方将无法为乙方进
行居留许可申请并意味着聘用关系的提前终止。同时，乙方入境后需进行健康检查和警察局
面谈，如因身体原因或在中国境内
该协议经双方签署后开始生效，
有效期最长为
6
个月，
但到双方签
署正式聘用合同时终
止。
