子宫腺肌病是由于子宫内膜间质和腺体进入子宫肌层而形成的
一种良性病变。
痛经和月
经量增多为主要表现。
对于年轻或有生育要求的患者可选择药物治疗;
无生育要求且病变广
泛、症状重、保守治疗无效者可选手术治疗;
痛经症状突出者可行保守手术
;子宫动脉栓塞
术、
腹腔镜下子宫动脉阻断术和强度聚焦超声<

(HIFU)
消融术也越来越多的应用到子宫
腺肌病的治疗。免疫分子也同时反应疾病的发生发展过程。近年来

医学界已将其划分为一
种独立的子宫疾病

。发病率为
25%

40%
,近年来有明显上升趋势[

。现已成为一
种育龄期妇女常见疾病。
目前在诊断与治疗方面

已取得很大进步及出现很多最新治疗进展。
本文就国内外诊疗方案综述如
下。
病因

子宫腺肌症病因不明确,与遗传因素、分娩或流产造成的
子宫损伤、慢性子宫内膜炎和
高雌激素血症等多种因素相关。多数研究者认为该病是由于
子宫内膜与肌层之间无黏膜下
层,
而内膜基底层腺体和间质侵及
内肌层或经脉管侵及外肌层所致。
一般子宫内膜腺体超过
子宫肌
层厚度的
1/4
(约

)即视为子宫腺肌症。
临床表现
